The Master on Bulk Carrier M.V Union Medal lost his life when a mooring line parted and hit him,causing his death.
At port Ilo, M.V Union Medal was Loading copper concentrate.
When one of the mooring line parted due to surging,
The Captain came on deck to evaluate the condition.
As he was assessing the situation second line broke off to strike him and killed him.
A study has shown that 20% of such accidents happens due to self negligence .
Due regards at all times must be paid to own safety.
